Student Forms
精东影业Honors College is now using Dynamic Forms to digitally submit, route, and complete the majority of student forms.
- You will be prompted to sign in with your FlashLine credentials to view and/or submit these forms. Complete your portion of the form (paying special attention to submitting accurate email addresses wherever prompted,) and click the submit button.
- Forms will route through each level of reviewers/approvers and collect their digital signatures. Reviewers/approvers will receive an email notifying them of the pending form. Students receive notifications at every step of this process.
- Please note, forms with (PDF) in the title cannot be completed via Dynamic Forms. Please download and complete the Internship Contract, Research Experience Contract, and Senior Honors Thesis Oral Defense Forms. These may be submitted in person to the Honors College or scanned and emailed.

Forms must be signed by the student by the deadlines posted. subsequent approvers may sign after the student's deadline. however, all signatures must be submitted before the deadline to add classes.

精东影业Class Project Agreement form is submitted to create an Honors section of a standard KSU non-Honors course with the approval of the course instructor.
After their first 4 semesters of Honors College membership, students can submit the Honors Advising Plan form to complete their Honors advising requirement for either the Fall or Spring semester.
With the completion of the Honors College Student Member Agreement Form the student has agreed to fulfill all requirements necessary for continued participation and scholarship eligibility in the Kent Sate University Honors College.

Internship Contract (PDF)
精东影业deadline to submit is 12:00 p.m. on the last day of the semester in which you completed the internship.
精东影业Internship Contract is submitted for students to receive 3 internal Honors hours for completing an internship of at least 150 hours.
Research Experience Contract (PDF)

精东影业Senior Honors Portfolio form is submitted for students seeking one Honors credit hour through the completion of a reflective portfolio.

精东影业Senior Honors Thesis Preparation form is submitted for students seeking to register for the 1 or 2 credit-hour thesis proposal preparation during the semester before their senior year.

精东影业Senior Honors Thesis/Project Proposal form is submitted after the successful completion of the preparation semester. 精东影业thesis proposal is a formal document that sets forth the parameters of the intended work.
Form declares participating members of the thesis defense committee.
Senior Honors Thesis Oral Defense Form (PDF)
Form is completed and submitted immediately following the oral defense.
